Conventions:

Single-qubit gates are boxes on one wire: $\boxed{H}$, $\boxed{T}$, $\boxed{S}$ - CNOT is a vertical line from the control qubit (solid dot $\bullet$) to the target qubit ($\oplus$) - Measurement is a meter symbol or $\boxed{M}$ - Classical wires (post-measurement) are drawn as double lines
