Appendix A · Medical Word Roots, Prefixes, and Suffixes

Medical vocabulary looks like an enormous memorization problem and is not. It is a small construction kit — perhaps 400 pieces — used over and over. Learn the pieces and you stop memorizing terms altogether: you read them, the way you read a sentence in a language you speak.

This appendix has two halves. The first teaches the skill of taking an unfamiliar term apart. The second is the parts bin: prefixes, roots organized by body system, and suffixes, each with at least one real term you will actually meet. Read the first half once, properly. Then keep the tables open and stop looking words up.


A.1 How to Decode a Term You Have Never Seen

Almost every medical term is built from three kinds of part.

  • The root (or combining form) carries the core meaning. It is almost always an organ, a tissue, a substance, or a process. Cardi- is heart. Nephr- is kidney. Hepat- is liver.
  • The prefix modifies the root, and it does so in a small number of predictable ways: number, position, direction, negation, time, speed, size, or color. Peri- is around. Hyper- is above or excessive. A- is without.
  • The suffix tells you what kind of thing the word names — a condition, a procedure, a specialty, or simply an adjective. -itis is inflammation. -ectomy is surgical removal. -ology is the study of.

A term needs a root. It may or may not have a prefix. It nearly always has a suffix.

The combining vowel

Roots are conventionally written with a slash and a vowel: cardi/o, nephr/o, gastr/o. That vowel — usually o, occasionally i — is a joint, and it follows one rule:

Keep the combining vowel when the next part begins with a consonant. Drop it when the next part begins with a vowel.

So cardi/o + -logy keeps the vowel, because l is a consonant: cardiology. But cardi/o + -itis drops it, because i is a vowel: carditis. The vowel is kept between two roots regardless — gastr/o + enter/o + -logy gives gastroenterology, vowel retained even though e is a vowel, because a word built of stacked roots would be unreadable otherwise. That single exception is worth knowing; it is why the long words have so many o's.

The vowel is not decoration. It exists because these words have to be said, and Greek and Latin do not tolerate the consonant clusters that direct joining would produce. "Cardlogy" is not pronounceable. "Cardiology" is.

Read the word backwards, then forwards

The efficient order is:

  1. Suffix first. It tells you what category of thing you are looking at. If the word ends in -ectomy, you already know somebody took something out, and the rest of the word only has to tell you what.
  2. Beginning next. The prefix, if there is one, sets position, number, or degree.
  3. Middle last. The root or roots, read left to right, in order.

Try it on subhepatic. Suffix -ic, an adjective — so this word describes something. Prefix sub-, beneath. Root hepat, liver. Beneath the liver. A subhepatic abscess is a pocket of pus sitting under the liver, and you knew that before anyone told you.

Why there are two words for everything

Medical English draws on both Greek and Latin, and it uses them for different jobs. Latin roots tend to name plain anatomy; Greek roots tend to build the words for disease and procedure. That is why the kidney is renal in an anatomy lab and nephrectomy in an operating theatre. It is not inconsistency — it is two layers of borrowing, deposited centuries apart.

Seven worked examples, in increasing difficulty

1 · pericarditis peri- (around) + cardi (heart) + -itis (inflammation) = inflammation of the tissue around the heart. That tissue is the pericardium, the fibroserous sac described in §1.7. The decode also predicts the clinical picture: an inflamed serous membrane rubs, so you hear a friction rub; an inflamed sac may weep fluid, so the cavity can fill and compress the heart (Chapter 18).

2 · gastroesophageal gastr/o (stomach) + esophag (esophagus) + -eal (pertaining to). Pertaining to the stomach and the esophagus — specifically to the junction between them. Note the order: the roots are listed in the direction of the process being described. In gastroesophageal reflux, contents travel from stomach to esophagus, and the word says so.

3 · hepatosplenomegaly hepat/o (liver) + splen/o (spleen) + -megaly (enlargement) = enlargement of the liver and spleen together. Three parts, no prefix. The reason the two organs are named in one word is physiological: they share the portal venous circulation, so a condition that raises portal pressure enlarges both (Chapter 23).

5 · hypercholesterolemia hyper- (above normal, excessive) + chol/e (bile) + ster (solid, from stereos) + -ol (an alcohol, in the chemical sense) + -emia (blood condition) = an excess of cholesterol in the blood. Five pieces. Notice that the middle of the word is itself a chemical name that was built the same way: cholesterol was named because it was first isolated, as a solid alcohol, from gallstones (Chapter 24).

6 · esophagogastroduodenoscopy esophag/o + gastr/o + duoden/o + -scopy (process of visual examination). Looking at the esophagus, then the stomach, then the duodenum — in exactly the order the scope travels. The word is a map of the procedure. Everyone calls it an EGD, but only because the full form is long, not because it is unclear.

7 · thrombocytopenia thromb/o (clot) + cyt/o (cell) + -penia (deficiency) = a deficiency of the cells that form clots, i.e. of platelets. Follow the decode to the consequence and you have the whole clinical syndrome: too few clotting cells means bleeding, and bleeding into skin gives the pinpoint hemorrhages called petechiae (Chapter 17).

Traps

Roots that look alike and are not. These pairs generate the majority of student errors, and several of them generate real clinical errors too.

Plurals. Medical English keeps its Greek and Latin plurals, and the rules are regular.

Two of these carry information you will use: meninx is singular, so the three-layered covering of the brain is properly the meninges (Chapter 12); and phalanx is singular, so a single finger bone is a phalanx and the set is phalanges (Chapter 7).

Clinical Connection · Why the Decode Is a Safety Feature

Look-alike terms are a documented source of medication and procedure error, and the defence is not memory but parsing. Three real examples:

  • Hyperkalemia and hypokalemia differ by two letters and demand opposite treatments — one calls for calcium, insulin with glucose, and potassium removal; the other calls for potassium replacement. Reading hyper- and hypo- as meaning-bearing prefixes rather than as word-shapes is what keeps them apart at 3 a.m. (Chapter 31).
  • Ileum and ilium are pronounced identically. An ileal conduit is built from bowel; an iliac graft is taken from bone. Spelling is the only signal, so the spelling matters.
  • Dysphagia and dysphasia are one letter apart and both common after stroke. One means the patient cannot swallow safely and needs their diet modified before they aspirate; the other means they cannot produce or comprehend language. A single mistyped letter sends the wrong specialist (Chapter 12).

The habit that prevents all three is the same: when a term matters, split it before you act on it.


A.2 Prefixes

A prefix never carries the core meaning of a term. It adjusts it — in number, position, negation, color, size, or time. That is the whole job, and it is why prefixes are worth learning first: there are few of them and they recur endlessly.

The -itis / -osis distinction. -itis asserts inflammation, which means immune cells and the vascular response are present. -osis asserts only an abnormal condition, usually degenerative or accumulative, with no claim about inflammation. This is why tendinosis and tendinitis are different diagnoses of the same tendon, and why calling a degenerative problem an "-itis" leads to treating it with anti-inflammatories that cannot help (Chapter 10).

Clinical Connection · Three Suffixes That Change the Diagnosis

  • -emia versus -uria. Bacteremia means organisms are circulating in the blood and is a medical emergency. Bacteriuria means organisms are in the urine and, in an asymptomatic patient, is often left alone entirely. The root is identical; the suffix is the whole clinical difference (Chapter 26).
  • -plegia versus -paresis. -plegia is complete paralysis; paresis is weakness. A patient described as hemiplegic cannot move the affected side; a hemiparetic patient can move it against gravity but not against resistance. Rehabilitation prognosis differs sharply (Chapter 12).
  • -rrhage versus -rrhea. Both mean flow, but -rrhage implies a burst — an abnormal, usually rapid loss of blood — while -rrhea implies a discharge that may be entirely normal. Menorrhagia and rhinorrhea live in different worlds.

Aging · The Vocabulary of Time

Several roots and suffixes exist chiefly to describe change with age, and you will meet them throughout Part V. Presby- means old age: presbyopia is the loss of near focus as the lens stiffens after about 45, and presbycusis is the high-frequency hearing loss of the ageing cochlea — Adwoa Mensah has both (Chapter 15). -osis attached to a tissue name usually signals a degenerative rather than inflammatory process, which is why the age-related conditions are osteoporosis, arthrosis, sclerosis, and atherosclerosis rather than "-itis" words. Sarco- plus -penia gives sarcopenia, the age-related loss of muscle mass that begins in the fourth decade and accelerates after 60 (Chapter 30). The morphology of the word tells you the morphology of the disease.

  1. myelopathymyel/o (spinal cord) + -pathy (disease). Literally: disease of the spinal cord. Clinically, any non-traumatic disorder of cord function — most often compression by degenerative cervical spine disease — producing weakness, spasticity, and a sensory level below the lesion (Chapter 12). Note the trap: myopathy is muscle, not cord.
  2. oliguriaolig/o (scanty) + -uria (urine condition). Literally: scanty urine. Clinically, urine output below roughly 400 mL/day in an adult (or under 0.5 mL/kg/h), a cardinal sign of acute kidney injury or of inadequate renal perfusion (Chapter 26).
  3. thrombocytopeniathromb/o (clot) + cyt/o (cell) + -penia (deficiency). Literally: a deficiency of clot cells. Clinically, a low platelet count; bleeding risk rises sharply below about 50,000/µL and spontaneous bleeding becomes likely below 10,000/µL (Chapter 17).
  4. polydipsiapoly- (many, much) + dips/o (thirst) + -ia (condition). Literally: much thirst. Clinically, one of the classic triad of untreated diabetes mellitus alongside polyuria and polyphagia — and the decode explains the link, since osmotic water loss in the urine is what drives the thirst (Chapter 16).
  5. dysphagiadys- (difficult) + -phagia (swallowing): difficulty swallowing, a risk for aspiration. dysphasiadys- + -phasia (speech): impaired language. One letter, two entirely different referrals (Chapter 12).
  6. hemiparesishemi- (half) + paresis (weakness). Literally: weakness of one half of the body. Clinically, partial weakness on one side, typically contralateral to a cerebral lesion; contrast hemiplegia, which is complete paralysis of that side.
  7. pyelonephritispyel/o (renal pelvis) + nephr (kidney) + -itis (inflammation). Literally: inflammation of the renal pelvis and kidney. Clinically, an upper urinary tract infection with fever and flank tenderness, distinguished from simple cystitis, which is confined to the bladder (Chapter 26).
  8. splenomegalysplen/o (spleen) + -megaly (enlargement). Literally: an enlarged spleen. Clinically it points to portal hypertension, hematological disease, or infection, and a spleen large enough to palpate below the left costal margin is at least twice normal size (Chapter 20).
  9. hypernatremiahyper- (above normal) + natri (sodium) + -emia (blood condition). Literally: high blood sodium. Clinically, serum sodium above about 145 mEq/L, which is nearly always a water deficit rather than a sodium excess — a distinction the word itself does not make, and one worth holding onto (Chapter 31).
  10. tachypneatachy- (rapid) + -pnea (breathing). Literally: rapid breathing. Clinically, a respiratory rate above 20 breaths/min in an adult. Amara Osei's rate of 24 in Case File 1 is tachypnea (Chapter 22).
  11. arthrodesisarthr/o (joint) + -desis (surgical fusion). Literally: binding of a joint. Clinically, the deliberate surgical fusion of a joint to abolish a painful, unstable articulation — trading movement for stability (Chapter 7).
  12. leukocytosisleuk/o (white) + cyt/o (cell) + -osis (abnormal increase). Literally: an increased condition of white cells. Clinically, a white cell count above about 11,000/µL, most often reactive to infection, inflammation, or stress (Chapter 17).
  13. retroperitonealretro- (behind) + periton (peritoneum) + -eal (pertaining to). Literally: pertaining to behind the peritoneum. Clinically, the position of the kidneys, ureters, adrenal glands, pancreas, and abdominal aorta — which is why these organs are approached surgically from the back or flank (§1.7).
  14. encephalomyelitisencephal/o (brain) + myel (spinal cord) + -itis (inflammation). Literally: inflammation of brain and spinal cord together. Clinically, a diffuse inflammatory disorder of the entire central nervous system (Chapter 12).
  15. cholecystojejunostomycholecyst/o (gallbladder) + jejun/o (jejunum) + -stomy (surgical opening between). Literally: a surgically created opening between the gallbladder and the jejunum. Clinically, a bypass performed when the bile duct is obstructed, diverting bile directly into the small intestine (Chapter 23). Four pieces, one procedure, and you could describe the operation without ever having seen it.
