Case Study 34.1 — Hernandez's Capstone

Why this case: the self-review that prevents the A1's problem
The capstone's threshold concept — reviewing your own proposal as a reviewer is the last and best revision — is best shown by watching it prevent a known failure. We know from Chapter 22 that Hernandez's R01, in one version, was a near-miss on the A0 because of a recruitment-feasibility weakness the reviewers caught, requiring a funded A1. This case shows the other version: Hernandez conducting a genuine reviewer's-eye self-review in the capstone, catching that same weakness before the real panel — the review before the review.
Assembly: integration, not collation
Hernandez first assembles her complete R01 package (Section 34.3): specific aims, research strategy (significance, innovation, approach), the rigor and inclusion components (Chapter 16), human-subjects and DMS plans, biosketches, budget and justification, letters. As she assembles, she checks coherence — and catches a mismatch: her approach references a preliminary-data figure her biosketch's productivity doesn't quite support. She aligns them. Assembly is integration: the cross-component check surfaces what separate drafting hid.
The role-shift: from author to reviewer
Now the capstone's central move. Hernandez sets the package aside for a day to break her author's immersion, then returns — not as the author who wrote it, but as a study-section reviewer who will judge it (Section 34.2). She opens the NIH review criteria and prepares to score herself honestly, as the panel will.
The criteria-based self-review
Reading critically against each criterion (Section 34.4):
- Significance: Strong. The diabetes-adherence problem is important; her case is compelling. A reviewer would score this well.
- Investigator(s): Strong. Her biosketch and team support it.
- Innovation: Solid.
- Approach: Here, reading as a skeptic, she hesitates. Would a critical reviewer believe her recruitment plan? She reads it as the reviewer would — and finds it thinner than she'd like. The single-site reliance, the enrollment assumptions — a reviewer would flag a feasibility doubt. As the author, she'd glossed it ("I know we can recruit"); as the reviewer, she sees the gap.
- Environment: Strong.
Her honest self-review has surfaced the recruitment weakness — the very weakness that, uncaught, drove the A0's near-miss and required the A1 (Chapter 22). But here she's caught it before the real panel, while she can still fix it.
The revision
Hernandez strengthens exactly what her reviewer's-eye review found: she adds a second clinical site with a commitment letter and pilot recruitment data demonstrating feasibility (the same fixes that, in the other version, she had to make in the A1 — now made in the A0). Her approach is now one a reviewer would believe. The reviewer's-eye self-review converted her own critical reading into the precise revision the proposal needed.
External review and the three passes
Hernandez extends her self-review with a mock review panel (Section 34.5) — colleagues reading critically against the NIH criteria, scoring, discussing — which confirms the recruitment fix and catches a minor clarity issue. She then runs the three-pass revision (Section 34.6): substantive (the recruitment strengthening, the clarity fix), compliance (the NIH format, page limits, required components), and polish (one clear voice, unmistakable significance and approach). Her R01 reaches submission readiness — and this time, the recruitment weakness that would have caused a near-miss is already fixed.
What this case teaches
- The reviewer's-eye self-review is the last and best revision. Becoming her own study-section reviewer let Hernandez find the recruitment weakness before the real panel.
- The self-review can prevent the A1's problem. The very weakness that drove a near-miss and required the A1, caught in the capstone, is fixed in the A0.
