Background

The WHO responded to this dual crisis — a pandemic of disease and a pandemic of misinformation — by coining the term "infodemic." Director-General Tedros Adhanom Ghebreyesus stated: "We're not just fighting an epidemic; we're fighting an infodemic. Fake news spreads faster and more easily than this virus, and is just as dangerous."
The COVID-19 infodemic was not simply an outbreak of lies. December 2020: FDA grants Emergency Use Authorization to the Pfizer-BioNTech vaccine. The Center for Countering Digital Hate (CCDH) publishes its "Disinformation Dozen" report, identifying twelve social media accounts responsible for approximately 65% of vaccine misinformation circulating online. They simply applied their existing infrastructure — their audiences, their posting strategies, their knowledge of which content performed best — to a new topic that was receiving unprecedented public attention. The head start that misinformation had over accurate public health information in the early pandemic was not incidental; it was the product of years of algorithmic amplification of anti-vaccination content.
The Disinformation Dozen: Concentration and Responsibility
The CCDH finding that twelve accounts were responsible for approximately 65% of vaccine misinformation online is one of the most practically significant findings in misinformation research. It suggests that the vaccine misinformation problem, while vast in scale, was concentrated in ways that should have made targeted enforcement feasible.

What Worked and What Didn't
The COVID-19 pandemic produced an unprecedented natural experiment in platform misinformation interventions. The evidence is nuanced.

Novel and emotionally arousing health information is not thereby false — but novelty and emotional intensity are not evidence of accuracy, and the engagement-optimization system surfaces them regardless of accuracy.
Identify institutional sources: For health decisions with significant consequences, identify the relevant authoritative institutions — national health agencies, peer-reviewed literature, established medical organizations — and seek their guidance directly, rather than through social media intermediaries who may have introduced distortion.
Understand the correction asymmetry: Research consistently shows that corrections of health misinformation have smaller effects than the original misinformation. If you share health information that you later discover is false, a correction to the same audience will not fully reverse the effect. This asymmetry argues for caution before sharing health claims, rather than relying on corrections after the fact.
Recognize network effects: The recommendation pathways that moved users into COVID-19 misinformation clusters were visible in retrospect — anti-vaccination content recommended alongside wellness content, vaccine-skeptical accounts recommended after following natural health accounts. Awareness of these pathways, and of how recommendation systems work, helps users recognize when they are being moved along them.
