In 1981, Barry Marshall and Robin Warren tried to tell the world that stomach ulcers were caused by a bacterium. The world told them they were wrong. None of these mechanisms require malice. None require stupidity. That is precisely what makes them dangerous. The failure modes documented in Part I operate on brilliant people in well-functioning institutions following the rules of their disciplines exactly as they were taught. The architecture of error is not a story about bad actors. It is a story about structural vulnerabilities in the way human beings produce, evaluate, and transmit knowledge -- vulnerabilities that become visible only when you know what to look for. By the end of these eight chapters, you will know what to look for. And you will begin to see the same patterns everywhere.
