26.1 The Domain of Public Health: Why It Is Especially Fertile for Propaganda

Public health communication occupies a unique position in the landscape of institutional messaging. Unlike political communication — which openly advocates for positions — public health communication presents itself as purely informational, neutral, and in the listener's interest. Its authority rests not on democratic legitimacy but on epistemic legitimacy: we should believe public health authorities because they tell the truth about things that matter for our survival. This gives public health communication its extraordinary power. It also gives anti-public-health propaganda its extraordinary target.

To understand why the public health domain is especially susceptible to propaganda, it helps to identify what makes any domain susceptible. Propaganda flourishes where:

  1. The stakes are high enough to motivate emotional engagement. Nothing raises stakes higher than threats to health and life. Public health crises — pandemic, epidemic, environmental contamination, drug safety failure — activate the deepest registers of human fear. Emotionally activated audiences are more susceptible to motivated reasoning and less capable of systematic evidence evaluation.

  2. The subject matter involves genuine complexity. Scientific research operates in uncertainty. Epidemiology involves probabilities, confidence intervals, and provisional conclusions that change as evidence accumulates. This genuine complexity creates linguistic raw material for propaganda: the doubt manufacturer does not need to fabricate uncertainty, only to amplify and misrepresent the uncertainty that already exists.

  3. The audience lacks independent verification capacity. Most people cannot read a clinical trial, evaluate a meta-analysis, or assess the methodology of an epidemiological study. This is not ignorance in any pejorative sense; it reflects the specialization of modern knowledge. But it creates a fundamental dependency: laypeople must rely on experts they trust rather than evidence they can evaluate. This trust dependency is the specific thing that anti-science campaigns target.

  4. The institutional communicators have historically erred. Public health institutions have made documented errors — the Tuskegee experiments, thalidomide, the initial dismissal of HIV in certain communities, repeated miscommunications during COVID-19. These errors are legitimate grounds for scrutiny. They are also raw material for a propaganda strategy that uses legitimate criticism of institutional failures to generalize distrust of institutional expertise.

  5. Personal identity is implicated. Health decisions are deeply personal. Decisions about vaccination, diet, medication, and environmental risk intersect with cultural identity, religious belief, parental anxiety, and political affiliation. When health decisions become identity markers — when being unvaccinated signals membership in a community — the persuasion challenge becomes an identity challenge, which is categorically harder to address.

The result is a domain in which the propaganda practitioner has access to an unusually powerful toolkit: health anxiety as emotional fuel, genuine scientific complexity as raw material for doubt, trust dependency as a structural vulnerability, institutional error as legitimate ammunition, and identity as armor against correction.


26.2 The Spectrum: Legitimate Public Health Communication to Anti-Science Propaganda

Before analyzing anti-science propaganda in the public health domain, it is essential to establish the analytical distinction between legitimate public health communication and propaganda. This is not always a clean distinction, but the key markers are identifiable.

Legitimate public health communication is characterized by:

  • Accuracy: Claims are based on the best available evidence and acknowledge genuine uncertainty honestly.
  • Transparent sourcing: The basis for claims is disclosed and verifiable. References to studies, institutional guidelines, and expert consensus are specific and checkable.
  • Transparent motivation: The communicator's purpose — to protect public health — is declared and consistent with the content. There is no hidden agenda served by the communication.
  • Self-correction: Legitimate communicators update their claims when evidence changes. The revision of COVID-19 masking guidance, for example, reflected genuine evidence evolution rather than strategic messaging.
  • Public interest orientation: The communication serves the health of audiences, not the financial or political interests of the communicator.

Anti-science propaganda in the public health domain is characterized by:

  • Manufactured doubt: Creating the appearance of scientific uncertainty where scientific consensus exists. This is the signature technique — not outright denial but the strategic exploitation and amplification of marginal dissent.
  • False expertise: Elevating contrarian voices with credentials insufficient to their claimed authority, while suppressing the weight of expert consensus.
  • Hidden motivation: Propaganda campaigns typically serve financial or political interests that conflict with the public health interests they claim to serve.
  • Exploiting health anxiety: Using fear of illness, medical authority, pharmaceutical industry, or government to make audiences receptive to claims that serve the propagandist's interests.
  • Conspiracy framing: Presenting the scientific consensus as the product of corruption or suppression rather than evidence accumulation.
  • Identity targeting: Attaching health skepticism to political, cultural, or religious identities to build durable communities of resistance to public health messaging.

The spectrum between these poles is real. Public health communication can be inaccurate without being propaganda. It can be oversimplified without being deceptive. It can serve institutional interests that partially diverge from public health interests without being manipulative. The student of propaganda must resist the binary and think about degree, intent, and effect. But the poles exist and the difference between them is morally significant. The Frank Statement — analyzed in detail below — represents the extreme: communication crafted to appear legitimate while deliberately undermining the public health it claimed to serve.


26.3 The Big Tobacco Model: Manufacturing Doubt in the Public Health Domain

The Complete Arc, 1950–2006

In 1950, two epidemiological studies — by Doll and Hill in the United Kingdom and Wynder and Graham in the United States — published evidence establishing a statistical link between cigarette smoking and lung cancer. The studies were not the first to suggest the connection, but they were the first to achieve the methodological rigor that positioned them as the foundation of an emerging scientific consensus.

The tobacco industry understood immediately what this meant. Not what it meant for public health — though the implications for public health were staggering, as executives would later document in their private correspondence — but what it meant for business. If smoking caused cancer, and the public came to believe it, the industry would face regulatory constraint, civil liability, and declining sales. The strategic question was not how to make cigarettes safer. The strategic question was how to prevent the scientific consensus from having the policy and commercial consequences that scientific consensus normally has.

The answer to that strategic question was developed in December 1953, when the chief executives of the major American tobacco companies met with the public relations firm Hill & Knowlton. The meeting produced a strategy that would remain in continuous operation for more than fifty years and that would serve as the template for virtually every major corporate anti-science campaign that followed.

The strategy had two components. The first was the manufacture of apparent scientific controversy. The industry would fund researchers willing to produce studies that questioned the cancer link, fund institutes to publish alternative explanations for lung cancer rates, and ensure that these contrarian voices received maximum visibility in media coverage. The goal was not to disprove the cancer link — the internal documents make clear that industry scientists believed the cancer link was real — but to create the appearance of ongoing scientific debate. If reporters, legislators, and laypeople perceived that scientists disagreed about whether smoking caused cancer, the effective policy consequence of the emerging consensus would be neutralized.

The second component was the colonization of public health discourse. Rather than opposing public health communication from the outside, the industry would speak the language of public health, express concern for consumers, commit to research, and position itself as a responsible participant in the project of protecting public health. This was not merely cynical — though it was deeply cynical — it was strategically sophisticated. An industry that appeared to be engaged with the health question was harder to regulate than one that appeared to be ignoring it.

The Internal Documents

The tobacco industry's internal documents, now available through the UCSF Truth Tobacco Industry Documents archive (a database of more than 14 million documents produced through litigation and congressional investigations), constitute one of the most comprehensive records of a sustained propaganda campaign that exists. They are extraordinary primary sources because they document, in the industry's own words, the gap between public statements and private knowledge.

Several documents deserve particular attention:

The 1969 Brown & Williamson memo quoted at the opening of this chapter — "Doubt is our product, since it is the best means of competing with the body of fact that exists in the minds of the general public" — was written by an internal strategist discussing the industry's approach to the lung cancer question. Its author was not describing a fringe view; he was articulating the central strategic logic that had governed the industry's communications for fifteen years.

The 1972 RJ Reynolds memo stated explicitly: "Doubt is our product." The repetition across companies and documents is significant: this was not an individual insight but a shared strategy, developed and coordinated across competitors who in this domain had a unified interest.

The 1961 Tobacco Industry Research Committee documents show the internal awareness that the "research" the TIRC was funding was not scientific research in any meaningful sense but strategic communications product: designed to be cited, to generate headlines, and to delay regulatory action.

These documents are not the only evidence of the industry's strategy. The epidemiology of the campaign's effects tells its own story.

The Human Cost

The connection between the tobacco industry's manufactured doubt campaign and preventable deaths is not abstract or speculative. It is the subject of substantial scholarship.

From the time of the first clear epidemiological evidence in 1950 through the implementation of effective tobacco regulation — a process delayed for decades by the industry's campaign — an estimated 8 million people died from tobacco-related disease who would not have died if effective regulation had been implemented on the timeline that evidence, absent deliberate obstruction, would have supported. This estimate, developed by public health researchers including Robert Proctor at Stanford, attempts to isolate the deaths attributable specifically to the delay in regulation — the gap between when regulation could have happened and when it did happen.

Eight million deaths is not a number that can be absorbed into a chapter without acknowledgment. It is approximately the population of New York City. It is larger than the death toll of any single military conflict since World War II. It is the human cost of manufacturing doubt.

The Template and Its Adoption

The tobacco industry's manufactured doubt strategy was not merely used in the tobacco industry. It was explicitly studied, documented, and adopted by other industries facing similar regulatory pressures. The tobacco documents themselves show awareness that the strategy was generalizable; Hill & Knowlton and other public relations firms applied versions of it to multiple clients.

In their 2010 book Merchants of Doubt, historians Naomi Oreskes and Erik Conway documented the specific ways in which the tobacco doubt template was adopted by the fossil fuel industry to combat climate science, by the lead industry to combat evidence of childhood lead poisoning, by the chemical industry to combat evidence of pesticide harm, and by the food industry to combat evidence of sugar's role in cardiovascular disease. In each case, the structural elements were consistent: funding contrarian research, elevating marginal dissent, suppressing internal findings of harm, and colonizing scientific and policy discourse with the language of uncertainty.

The template's adoption was sometimes literal. Documents from the Global Climate Coalition — an industry group formed to combat climate regulation — show explicit references to the tobacco industry's model of managing scientific controversy.


26.4 The Anti-Vaccine Movement: From Wakefield to the Social Media Age

The Wakefield Fraud

On February 28, 1998, The Lancet published a study by British physician Andrew Wakefield and twelve co-authors claiming to find a potential link between the MMR (measles-mumps-rubella) vaccine and autism in twelve children. The study was small, the methodology weak, and the sample unrepresentative — problems that would have limited its significance under normal circumstances. But the Lancet imprimatur, combined with a press conference at which Wakefield called for the suspension of the MMR vaccine, produced an international media event.

What was not disclosed at the time: Wakefield had received £55,000 (later totaling over £435,000) from a law firm seeking to sue vaccine manufacturers. He had a patent application filed for an alternative measles vaccine that would benefit commercially from the discrediting of MMR. He had manipulated, according to subsequent investigation by journalist Brian Deer, the clinical data of the twelve children in the study — in some cases dramatically misrepresenting the timeline of symptoms in ways that made the vaccine-autism link appear where it did not exist in the actual medical records.

The Lancet retracted the paper in 2010. The UK General Medical Council found Wakefield guilty of serious professional misconduct and struck him from the medical register. A dozen subsequent large-scale studies, involving hundreds of thousands of children, found no link between MMR vaccination and autism.

Andrew Wakefield moved to Austin, Texas, became a prominent figure in the American anti-vaccine movement, produced a documentary alleging CDC vaccine fraud, and retained a significant following. As of this writing, he remains a major voice in vaccine skepticism and was active in COVID-19 vaccine opposition.

The Propaganda Anatomy

The Wakefield episode established the anti-vaccine movement's propaganda toolkit, which has remained remarkably consistent across twenty-five years:

Manufactured scientific controversy. A single fraudulent study was treated in media coverage as establishing genuine scientific debate where none existed. The "balance" norm of journalism — presenting "both sides" — meant that Wakefield's claims received equivalent weight to the consensus of the global epidemiological community for years after the original retraction.

False expertise. Wakefield held genuine medical credentials, which gave him initial legitimacy. As his fraud was exposed, the movement shifted to promoting a rotating cast of physicians, researchers, and scientists whose credentials were real but whose claims were unsupported by their actual area of expertise. A radiologist speaking about vaccine immunology, a retired researcher promoted as having "inside knowledge" — the form of expertise was maintained while its substance was hollowed out.

Conspiracy framing. The retraction of the Wakefield paper and the loss of his medical license were framed not as consequences of documented fraud but as evidence of pharmaceutical industry suppression. This is a structurally closed argument: every piece of evidence against the vaccine-autism link becomes, within the conspiracy frame, evidence of the conspiracy. The discrediting of Wakefield becomes proof that he was getting too close to the truth.

Parental anxiety exploitation. Autism diagnoses increased during the same period that childhood vaccination schedules expanded — a correlation that is attributable to expanded diagnostic criteria rather than causation, but that created a pool of parents searching for explanations for their children's diagnoses. The anti-vaccine movement offered a clear causal narrative, a villain (pharmaceutical companies, complicit regulators), and a community of shared experience. This is not a cynical observation: genuine parental grief and love were channeled by a propaganda movement toward conclusions that the evidence did not support.

Social media amplification. Anti-vaccine networks developed robust online infrastructure well before COVID-19. The Center for Countering Digital Hate identified what it called the "Disinformation Dozen" — twelve accounts responsible for approximately 65% of anti-vaccine content on social media platforms in 2021. These networks, built over years on Facebook, YouTube, and Twitter, were immediately activated for COVID vaccine opposition when vaccines became available.

The Measles Return

The measles outbreaks of 2019 — the worst in the United States since the disease was declared eliminated in 2000 — are the direct epidemiological consequence of the Wakefield fraud's long tail. Measles vaccination rates fell in communities where anti-vaccine sentiment was concentrated; the disease, for which a highly effective vaccine exists, spread. The 1,282 cases reported in the U.S. in 2019 — a disease that had been eliminated — represent the human cost of the anti-vaccine propaganda campaign in a single year, in a single country.


26.5 Climate Science Denial as a Public Health Issue

Climate change is sometimes categorized as an environmental issue rather than a public health issue. This framing is a significant analytical error that serves the interests of those who wish to delay climate action. The connections between climate change and public health are extensive and well-documented: increased mortality from extreme heat events; expanded range of vector-borne diseases (malaria, dengue, Lyme disease); air quality degradation from wildfire smoke; drought-driven food insecurity and undernutrition; coastal flooding and displacement with associated mental health consequences.

This means that climate science denial is, among other things, a public health propaganda campaign. By sustaining doubt about the scientific consensus on climate change, and by delaying the policy responses that the scientific evidence calls for, the fossil fuel industry's decades-long manufactured doubt campaign has contributed to health harms at a scale that rivals the tobacco campaign.

The Explicit Adoption of the Tobacco Template

Oreskes and Conway's Merchants of Doubt documents the specific ways in which the fossil fuel industry adopted the tobacco model, including direct institutional connections. Several of the key figures in both campaigns were the same: Frederick Seitz, who chaired the Tobacco Industry Research Committee in the 1960s and 1970s, later played a leading role in creating the appearance of scientific controversy about climate change through the George C. Marshall Institute.

The Global Climate Coalition, formed in 1989 as a lobbying group for fossil fuel interests, produced internal documents showing that its members' own scientists had confirmed the scientific consensus on human-caused climate change. The organization's public communications denied that consensus. The internal/external gap is structurally identical to the tobacco industry's internal acknowledgment of the cancer link alongside public manufactured doubt.

The Propaganda Infrastructure

The fossil fuel industry's climate denial campaign created a substantial institutional infrastructure:

The Heartland Institute is a Chicago-based think tank that has received significant fossil fuel funding and has been the primary organizer of the Nongovernmental International Panel on Climate Change (NIPCC), a shadow version of the Intergovernmental Panel on Climate Change designed to produce contrarian scientific literature. The NIPCC's reports — which dispute IPCC findings — are formatted to visually mimic IPCC reports, a design choice that serves confusion rather than clarity.

Op-ed placement campaigns. Internal documents from ExxonMobil show a coordinated campaign to place opinion pieces questioning climate science in major newspapers. The campaign was specifically designed to use the authority of scientific language while obscuring the financial interests behind the claims.

The Koch network. Extensive investigative reporting, most comprehensively by Jane Mayer in Dark Money, has documented the role of Charles and David Koch's network of foundations and advocacy organizations in funding climate denial infrastructure. The network's financial connections run through Heartland, Americans for Prosperity, the Competitive Enterprise Institute, and dozens of other organizations that consistently produced contrarian content on climate science.

The public health consequence of this campaign's success — measured in delayed climate policy, continued fossil fuel combustion, and the health harms that result — is incalculable but unambiguously large.


26.6 The Food Industry and Nutrition Science Propaganda

In 2016, a research team led by Cristin Kearns published a paper in JAMA Internal Medicine that produced, in the words of the New York Times, "a bombshell." Kearns and her colleagues had analyzed internal documents from the Sugar Research Foundation — an industry group funded by the sugar industry — and found clear evidence that the foundation had funded Harvard nutrition researchers in the 1960s and 1970s to produce studies that shifted blame for cardiovascular disease away from sugar and toward dietary fat.

The researchers paid for were not fabricating data in the Wakefield sense. They were selecting which questions to ask, which findings to emphasize, and which contrary evidence to minimize. The result was a body of published research that shaped nutrition science and public dietary guidelines for decades, contributing to the low-fat dietary paradigm that, as subsequent research has established, was based on misleadingly incomplete evidence about the cardiovascular effects of sugar consumption.

The Structural Parallel to Tobacco

The structural parallel to the tobacco model is precise:

  • An industry identified a scientific finding that threatened its commercial interests (evidence linking sugar to cardiovascular disease, as established by Yudkin and others in the 1960s).
  • The industry funded research designed not to refute the finding but to create the appearance of alternative explanations that would redirect regulatory and public attention.
  • The funded researchers had real credentials and published in real journals — the legitimacy of the form disguised the corruption of the intent.
  • The industry suppressed internal research that confirmed the link. (SRF internal documents show awareness of evidence the industry did not publish.)
  • The campaign succeeded: fat, not sugar, became the dietary villain of the last third of the twentieth century, a paradigm embedded in public health guidelines, food product formulation, and popular nutrition understanding.

Manufactured Uncertainty vs. Genuine Scientific Uncertainty

This case study requires an important analytical caveat that the student of propaganda must be able to make. Nutrition science is genuinely difficult. The methodological challenges of dietary research — the difficulty of controlled experiments, the complexity of dietary patterns, the long time horizons of nutritional effects — mean that genuine uncertainty is pervasive in nutrition science. The ongoing debates about optimal macronutrient ratios, the health effects of specific foods, and the relative contribution of diet to various diseases reflect real scientific challenges, not manufactured doubt.

The propaganda analysis does not eliminate the genuine uncertainty. What it identifies is the deliberate shaping of that uncertainty for commercial benefit — the strategic funding of research to ensure that the uncertainty landscape was tilted in industry-favorable directions. The distinction matters because it affects the remedy: the problem is not that we know more than is being communicated, but that what we know has been shaped by interests that do not align with public health.


26.7 The Opioid Crisis as Corporate Health Propaganda

On December 12, 1995, the U.S. Food and Drug Administration approved OxyContin, Purdue Pharma's formulation of oxycodone. The following decade saw one of the most consequential corporate propaganda campaigns in American public health history: a systematic effort by Purdue Pharma to market OxyContin using false claims that, when believed by physicians and patients, contributed to an addiction and overdose epidemic that killed hundreds of thousands of people.

The False Claims

Purdue Pharma's marketing of OxyContin rested on two central false claims:

The "less than 1% addiction rate" claim. Purdue's sales representatives were trained to tell physicians that fewer than 1% of patients who took OxyContin would become addicted. This claim was derived from a brief letter published in the New England Journal of Medicine in 1980 — not a study, but a letter reporting that hospital patients treated with narcotics rarely developed addiction. The letter said nothing about patients prescribed opioids for long-term outpatient pain management. The claim was not simply inaccurate; it was a deliberate misrepresentation of a source that did not support it.

The extended-release formulation claim. Purdue claimed that OxyContin's extended-release formulation made it less addictive than immediate-release opioids. This claim was also false. Internal company documents, produced through litigation, show that Purdue was aware by the late 1990s that OxyContin was being widely abused by crushing and snorting or dissolving the tablets — techniques that defeated the extended-release mechanism. The company continued to promote the addiction-safety claim while suppressing this awareness.

The Marketing Infrastructure

The OxyContin marketing campaign deployed resources and techniques that warrant description:

Physician targeting. Purdue assembled databases of physicians who prescribed opioids at high rates and directed its sales force specifically to them, providing incentives for prescribing that amounted to a systematic corruption of the physician-patient trust relationship. Physicians were provided with coupons for free OxyContin prescriptions and with gifts and entertainment in ways that federal prosecutors later characterized as an inducement scheme.

Key opinion leader co-optation. Purdue recruited prominent physicians to serve as paid speakers and endorsers, creating the appearance of independent expert endorsement for claims that the company had manufactured. This is the false expertise technique in its pharmaceutical form: real physicians with real credentials endorsing claims that their own expertise should have led them to reject.

Targeting of underserved communities. Internal Purdue documents, analyzed by Beth Macy in Dopesick and by Patrick Radden Keefe in Empire of Pain, show that the company specifically targeted Appalachian communities with high rates of physical labor injuries — populations with genuine pain, limited healthcare access, and few alternatives. The geographic concentration of the opioid epidemic's earliest devastation in rural Appalachia, Maine, and West Virginia is not coincidental; it is the direct result of a marketing strategy.

The Human Cost

At its peak, the opioid crisis killed more than 80,000 Americans per year — a number that surpassed annual traffic fatalities and homicides combined. The Centers for Disease Control and Prevention estimated that between 1999 and 2020, nearly 500,000 people died from opioid overdose in the United States. The relationship between Purdue Pharma's marketing campaign and this epidemic is not causal in a simple linear sense — the epidemic involved multiple opioid manufacturers, distributors, pharmacies, and physicians — but the causal role of Purdue's false claims in initiating and sustaining the prescription opioid phase of the crisis is documented in the company's own records and in federal guilty pleas.

The Sackler family, which owned Purdue Pharma, received approximately $10 billion in distributions from the company while the crisis unfolded. This juxtaposition — $10 billion in family wealth, 500,000 deaths — is not rhetorical excess. It is the arithmetic of the public health propaganda.


26.8 The COVID-19 Infodemic: Domain Analysis

The term "infodemic" — coined by author David Rothkopf in 2003 and adopted by the World Health Organization during COVID-19 — refers to the rapid spread of misinformation during a health crisis. The COVID-19 infodemic was not simply a volume problem; it was a structural feature of the media environment that specifically targeted the public health behaviors most essential to limiting the pandemic's mortality.

False Treatment Claims

Scientific consensus-building is a slow, methodologically conservative process: it requires replication, peer review, expert evaluation, and the accumulation of converging evidence across independent research programs. Manufactured doubt requires only the introduction of apparent uncertainty — a single study with questionable methodology, a credentialed scientist willing to dissent, a think tank report formatted to look like a review of the literature. The doubt machine does not need to win the scientific argument; it only needs to delay the regulatory consequence of the scientific argument. And delay, measured in years or decades of continued tobacco sales, fossil fuel combustion, sugar consumption, opioid prescriptions, and falling vaccination rates, is measured in deaths.
