The video alleged that the COVID-19 pandemic was an orchestrated plot by global elites, that vaccines were designed to implant tracking microchips, and that face masks caused disease rather than prevented it. YouTube removed the video. Facebook removed it. Yet millions of people had already downloaded and re-uploaded it to dozens of platforms, and copies continued circulating for months.
The Plandemic phenomenon was, by any measure, extraordinary. But it was not surprising to scholars who study conspiracy theories. The conditions that made Plandemic possible — a global crisis generating uncertainty and fear, a fractured media ecosystem, algorithmic amplification, and a pre-existing network of conspiratorial belief communities — had been building for years. Plandemic was not a cause. It was a symptom.
This chapter examines conspiracy theories systematically: what they are, why people believe them, how they spread, and what can be done in response. The subject demands rigor. Conspiracy theories are not merely intellectual curiosities or social eccentricities. They drive vaccine refusal, fuel political violence, undermine democratic governance, and destroy lives. Understanding them is not optional for citizens of the contemporary information environment. It is essential.
A critical preliminary point: not all conspiracy theories are false. Some conspiracies are real. The FBI's COINTELPRO program, which operated from 1956 to 1971, involved the systematic surveillance, infiltration, and sabotage of civil rights organizations, including the assassination of Fred Hampton by Chicago police in coordination with the FBI. The Tuskegee Syphilis Study, conducted by U.S. public health officials from 1932 to 1972, withheld treatment from Black men with syphilis without their knowledge or consent. The tobacco industry conspired for decades to suppress research linking cigarettes to cancer. These were real conspiracies, uncovered by journalists, whistleblowers, and investigators. Healthy skepticism toward institutional power is not pathological; it is rational.
What distinguishes pathological conspiracy belief from rational skepticism is not the object of suspicion — it is the epistemological structure of the belief. This distinction is the starting point for our analysis.
Section 13.1: Defining Conspiracy Theories
The Core Definition
Cass Sunstein and Adrian Vermeule, in their influential 2009 essay "Conspiracy Theories: Causes and Cures," define a conspiracy theory as "an effort to explain some event or practice by reference to the machinations of powerful people, who have also managed to conceal their role." This definition captures several essential elements: a causal explanation, an attribution to powerful agents, and crucially, a claim about concealment. The concealment element is what makes conspiracy theories resistant to standard epistemic practices: if the powerful actors have successfully hidden their tracks, then the absence of evidence becomes, paradoxically, evidence itself.
Sunstein and Vermeule's work is often discussed in terms of their controversial policy prescriptions — they proposed "cognitive infiltration" of conspiracy groups by government agents — but their definitional framework remains the most widely cited in the academic literature. The emphasis on the machinations of powerful people distinguishes conspiracy theories from ordinary causal explanations and from simple suspicion or distrust.
A more expansive definition comes from Rob Brotherton, whose 2015 book "Suspicious Minds: Why We Believe Conspiracy Theories" characterizes conspiracy theories as "an unverified and not necessarily false narrative that attributes the ultimate cause of an event or circumstance to a secret plot by multiple powerful actors working in concert." Brotherton's addition of "not necessarily false" is important: it preserves epistemic humility about historical real conspiracies while still allowing analysis of the category.
When Conspiracy Theories Are Correct
Before developing a critical framework for analyzing conspiracy theories, we must reckon honestly with the fact that some have turned out to be true. This is not merely a rhetorical concession; it is a structurally important point for understanding why blanket dismissal of conspiracy theories is epistemically inadequate.
COINTELPRO (1956-1971): The FBI's Counter Intelligence Program was a classified domestic surveillance program that targeted civil rights leaders, socialist organizations, and anti-war groups. The program involved mail interception, phone tapping, forged documents designed to sow internal dissension, and coordination with local law enforcement in operations that resulted in violence and death. At the time, allegations of such activities were widely dismissed as paranoid conspiracy thinking. The program was only exposed in 1971 when activists burglarized an FBI field office in Media, Pennsylvania, and leaked documents to the press. Congressional investigations (the Church Committee, 1975-1976) subsequently confirmed the program's scope and illegality. Those who had alleged FBI persecution of civil rights leaders were, it turned out, correct.
The Tuskegee Syphilis Study (1932-1972): The U.S. Public Health Service, in partnership with the Tuskegee Institute, enrolled 399 Black men with syphilis (and 201 without as controls) in a study of the "natural progression" of untreated syphilis. The men were not told they had syphilis; they were told they had "bad blood." When penicillin became the standard of care in 1947, it was deliberately withheld. The study continued until 1972, when a whistleblower leaked it to the press. Twenty-eight men died of syphilis directly; 100 died of related complications; 40 wives were infected; 19 children were born with congenital syphilis. The Tuskegee study has had lasting effects on Black Americans' trust in medical institutions — a legacy that profoundly complicated COVID-19 vaccination campaigns fifty years later.
The Tobacco Industry's Suppression of Cancer Research: Internal documents revealed through litigation in the 1990s confirmed that tobacco companies had known since the 1950s that cigarettes caused cancer and had orchestrated a decades-long campaign — including funding industry-friendly research, lobbying against regulation, and publicly denying established science — to obscure this fact. The "Merchants of Doubt" industry playbook developed in response to tobacco research has since been deployed by the fossil fuel industry, the sugar industry, and others.
These cases matter for at least three reasons. First, they establish that powerful actors do sometimes engage in genuine conspiracies, and that institutional skepticism is rational rather than paranoid. Second, they explain why communities with historical experiences of institutional betrayal — particularly African Americans — may show elevated suspicion of government health initiatives. Third, and most importantly for our purposes, they demonstrate that the distinction between legitimate skepticism and pathological conspiracy belief must be made on structural and epistemological grounds, not on the simple grounds that "powerful people don't conspire."
Barkun's Typology
Michael Barkun, a political scientist at Syracuse University, offers the most comprehensive typology of conspiracy theories in his 2003 book "A Culture of Conspiracy." Barkun identifies three types:
Event conspiracy theories allege that a specific, discrete event was the product of a conspiracy. The event in question is usually of significant social importance. The assassination of John F. Kennedy, the September 11 attacks, the death of Princess Diana, and the moon landing hoax narrative all fall into this category. Event conspiracy theories are the most common and often the most tractable: they involve falsifiable claims about specific events.
Systemic conspiracy theories posit that a conspiracy is ongoing and operates across a broad domain of social life — politics, economics, culture, or religion. The conspiracy is not directed at any single event but at achieving long-term control over institutions. Antisemitic narratives about Jewish control of media and finance, narratives about "globalist" agendas, and allegations of permanent deep-state governance are systemic conspiracy theories. These are more expansive than event theories and more resistant to refutation because they do not make specific predictive claims.
Superconspiracy theories are the most totalizing: they posit that multiple conspiracies are in fact connected, all directed by a single overarching power. QAnon, which alleges that a Satan-worshipping cabal of global elites runs governments, media, and finance, is the clearest contemporary example of a superconspiracy theory. Barkun notes that superconspiracy theories tend to be the most culturally resonant because they offer the most complete explanatory framework — a unified theory of evil that accounts for everything wrong in the world.
Barkun also observes that superconspiracy theories tend to evolve from the aggregation of event and systemic theories. Individual conspiracy narratives that might initially circulate independently are gradually woven together into larger frameworks. The anti-vaccination narrative, the 5G tower narrative, and QAnon's pedophile elite narrative have been integrated into a single supersystem by segments of the conspiratorial belief community.
Section 13.2: The Psychology of Conspiracy Belief
The Motivational Framework
Why do people believe conspiracy theories? Psychologists Karen Douglas, Robbie Sutton, and colleagues at the University of Kent have developed the most comprehensive motivational account in the literature. Their framework, presented in a series of papers from 2017-2022 and synthesized in Douglas et al. (2019), identifies three clusters of psychological needs that conspiracy theories serve.
Epistemic Motives: The Need for Certainty
Human beings have a powerful drive to understand the world around them. We seek explanations, causal accounts, and predictive frameworks. When events are ambiguous, threatening, or unexplained, this drive intensifies. Conspiracy theories offer the appearance of epistemic satisfaction: they provide explanations — often highly detailed ones — for puzzling or threatening events.
The need for cognitive closure — a construct developed by Arie Kruglanski — refers to the desire for a firm answer to a question as opposed to ambiguity. Research consistently finds that individuals with higher need for cognitive closure are more prone to conspiracy belief. Conspiracy theories appeal to this need because they eliminate ambiguity: there is always an explanation, and the explanation always has a clear villain.
Importantly, conspiracy theories offer what might be called "illusory epistemic superiority." Believers often feel they possess hidden knowledge that others lack — that they have "done the research" and can see what the masses cannot. This feeling of superior knowledge is psychologically rewarding and helps explain why conspiracy believers often feel contempt rather than sympathy for non-believers.
Brotherton's research highlights a related construct: the need for uniqueness. People who feel their knowledge and identity are distinctive may be drawn to conspiracy theories that mark them as epistemically special — as members of an enlightened minority who know the truth.
Existential Motives: Managing Threat and Anxiety
The second cluster of motives involves threat management. Conspiracy theories often emerge and intensify during periods of threat — natural disasters, pandemics, economic crises, social upheaval. The psychological function of conspiracy theories in these contexts is to transform an uncontrollable, diffuse threat into a locatable, bounded one.
If COVID-19 was released intentionally by the Chinese government or engineered by pharmaceutical companies, then the source of threat is identified and, in principle, addressable. If it is simply an emergent pathogen produced by the ordinary risks of ecological disruption in an interconnected world, the threat feels more diffuse and harder to manage. Conspiracy theories transform the anxiety of living in a complex, uncontrollable world into the more manageable anxiety of facing a specific, knowable enemy.
This threat-management function also explains the paradox that conspiracy theories can be simultaneously alarming and comforting. The content is frightening — secret cabals controlling global events — but the framework is reassuring because it imposes order on chaos. As Jan-Willem van Prooijen and Mark van Vugt argue in their 2018 paper on the evolutionary psychology of conspiracy beliefs, this may reflect an adaptive tendency: in ancestral environments, assuming agency (even mistakenly) behind threatening patterns was less costly than failing to detect genuine threats.
Social Motives: Belonging and Distinctiveness
The third cluster involves social needs. Human beings are intensely social creatures whose sense of self is bound up with group membership. Conspiracy theories function as identity markers. Believing a particular conspiracy theory aligns one with an in-group (those who know the truth) and against an out-group (the sheeple, the sleeping masses, the media propagandists).
Research by Roland Imhoff and Pia Lamberty finds that conspiracy beliefs correlate with the need for uniqueness — not mere social belonging, but distinctive belonging. Believers are not seeking the majority position; they are seeking a position that marks them as specially informed. This need for uniqueness may explain why conspiracy believers often resist evidence that their views are becoming mainstream: as QAnon-adjacent beliefs entered the Republican mainstream, some adherents moved to more extreme positions to maintain their sense of epistemic distinction.
Douglas and Sutton's work also establishes that conspiracy beliefs are associated with decreased trust in institutions — governments, media, science, corporations — and that this distrust is both a cause and a consequence of conspiracy belief. Initial distrust makes conspiracy theories more plausible; belief in conspiracy theories deepens distrust. This feedback dynamic helps explain why conspiracy beliefs can be so persistent.
Section 13.3: Cognitive Mechanisms
Proportionality Bias
One of the most robust findings in the psychology of conspiracy belief is proportionality bias: the intuition that big events must have big causes. If the event is significant — a presidential assassination, a global pandemic, the collapse of towers — the explanation must be equally significant. A lone gunman or a bat coronavirus emerging from a wet market seems inadequate as an explanation for world-historical events.
Proportionality bias appears to be a fairly general cognitive tendency, not restricted to conspiracy-prone individuals. Aaronson and Wenger (2009) found that participants were more likely to invoke conspiratorial explanations for high-consequence events than for identical events described as low-consequence. This suggests that proportionality bias is not simply a symptom of conspiratorial personality but a general feature of human causal reasoning that conspiracy theories exploit.
Pattern Overdetection and Apophenia
The human brain is exquisitely sensitive to patterns. This pattern-detection capacity is adaptive: it allows us to recognize food, predators, allies, and environmental regularities. But it also produces false positives — seeing patterns in noise, faces in clouds, meaningful sequences in random data. Psychologist Peter Brugger coined the term "apophenia" (later popularized by Klaus Conrad) to describe the spontaneous perception of connections between unrelated things.
Michael Shermer describes the relevant faculty as "patternicity" — the tendency to find meaningful patterns in both meaningful and meaningless data. Conspiracy theorists display a heightened sensitivity to apparent patterns: dates that seem significant, coincidences that seem impossible, events that seem too convenient. The internal logic of conspiracy thinking is often a form of apophenic reasoning in which coincidences become evidence of coordination.
Research by Viren Swami and colleagues has found that a tendency toward seeing patterns where none exist — measured using tasks such as identifying shapes in ambiguous images — correlates with higher scores on conspiracy belief measures. This is consistent with the apophenia hypothesis.
Agency Detection and HADD
Related to pattern detection is agency detection — the tendency to interpret events as caused by intentional agents rather than impersonal forces. Anthropologists and cognitive scientists have proposed that human beings have a hyperactive agency detection device (HADD) — a tendency to over-attribute agency that evolved because failing to detect an agent (a predator, an enemy) was more costly than falsely detecting one.
In the context of conspiracy theories, HADD manifests as the tendency to interpret bad outcomes as the result of someone's malicious intent rather than structural forces, bad luck, or complexity. When a pandemic kills millions, HADD pushes toward asking "who did this?" before "how did this happen?" Conspiracy theories satisfy HADD by providing an agent — always a powerful, malicious, and secretive one.
Monological Belief Systems
Perhaps the most structurally important cognitive feature of conspiracy thinking is what Stephan Lewandowsky and colleagues call the "monological" character of conspiracy beliefs. A monological belief system is one in which all beliefs support and reinforce each other, creating an internally consistent but hermetically sealed worldview.
Research by Lewandowsky et al. (2013) found that belief in one conspiracy theory is a strong predictor of belief in other, unrelated conspiracy theories — including mutually contradictory ones. Participants who believed that Princess Diana faked her own death were also more likely to believe that she had been murdered by the Royal family. Both cannot be true, yet both beliefs co-occurred. This is not mere logical inconsistency; it reflects a deeper epistemic stance in which "the official story is wrong" is the operative principle, regardless of what the alternative story is.
The monological quality of conspiracy belief means that refuting individual claims is often ineffective. Because beliefs are mutually reinforcing, the removal of one does not destabilize the system; the believer simply adjusts the network of supporting beliefs. This has important implications for debunking strategies, as we will explore in Section 13.8.
Section 13.4: Sociological Conditions
Marginalization and Powerlessness
Conspiracy theories are not evenly distributed across populations. Research consistently finds that they are more prevalent among groups that experience social marginalization, economic insecurity, and diminished political efficacy. This is not a reflection of intellectual capacity; it is a structural phenomenon rooted in the real experience of institutional failure.
Roland Imhoff and colleagues find that feelings of powerlessness — the sense that one lacks control over important outcomes — are among the strongest sociological predictors of conspiracy belief. When people feel that formal political and economic institutions are unresponsive to their needs, alternative explanatory frameworks become appealing. If the system is rigged, then conventional means of understanding and influencing it are inadequate; one needs to see behind the scenes.
Communities with historical experiences of institutional betrayal show elevated conspiracy belief levels that are, in many cases, rationally calibrated to their historical experience. Research by Sherrill Sellers and colleagues on COVID-19 vaccine hesitancy among African Americans found that hesitancy was substantially predicted by historical awareness of the Tuskegee study — a reasonable response to documented evidence of medical malfeasance, not a symptom of cognitive deficiency.
Rapid Social Change
Sociological research identifies rapid social change as a major facilitating condition for conspiracy belief. When social institutions change faster than people's frameworks for understanding them, the resulting disorientation creates fertile ground for explanatory systems that attribute the disruption to malicious intent. The conspiracy theory functions as a narrative technology for making sense of change that otherwise seems chaotic.
This pattern appears across historical periods. The Protocols of the Elders of Zion — a fraudulent antisemitic text fabricated in Russia in the early twentieth century — gained traction in a Europe experiencing rapid urbanization, industrialization, and nationalist upheaval. The John Birch Society's conspiracy theories about communist infiltration flourished in Cold War America during a period of dramatic social change. QAnon emerged and spread during a period of accelerating economic inequality, institutional delegitimation, and social media disruption of information markets.
Cross-Cultural Patterns
While conspiracy theories are sometimes discussed as a distinctively American phenomenon, research demonstrates that they appear across cultures globally. Jan-Willem van Prooijen and colleagues have conducted cross-national studies finding conspiracy belief in dozens of countries, with some consistent predictors (political extremism, distrust in institutions) and some culture-specific ones.
There are cross-cultural variations in which groups are cast as the conspiring villains. In some contexts it is global elites, in others ethnic minorities, in others foreign governments, in others religious groups. But the structural features of conspiracy theories — secret powerful actors, malicious intent, concealed evidence — appear across cultural contexts, suggesting that the underlying psychological and social conditions that generate them are fairly universal.
Section 13.5: The Anatomy of a Conspiracy Theory
Key Structural Features
Whatever their specific content, conspiracy theories share a set of structural features that give them their distinctive character. Understanding these features is essential for identifying conspiracy theories and for developing effective responses to them.
A secret, powerful group operating with malicious intent. Conspiracy theories always posit actors who are simultaneously powerful (capable of orchestrating and concealing large-scale operations) and secret (not openly visible in their operation). The group may be identified specifically (the Illuminati, the deep state, Big Pharma) or vaguely ("the elites," "they"). The secrecy is not incidental; it is constitutive of the conspiracy. Without concealment, there is no conspiracy — only a policy.
Nothing is coincidental; everything is connected. In a conspiracy theory, random events do not exist. Every coincidence is meaningful; every event is part of the plan. The conspiracy theorist's task is to decode the pattern — to reveal the hidden connections beneath the apparent randomness of events. This feature makes conspiracy theories unfalsifiable in practice: any event, however innocuous, can be incorporated into the explanatory framework.
Contradictory evidence confirms rather than refutes. This is perhaps the most structurally important feature of conspiracy theories. Because the conspirators are powerful and secretive, the absence of evidence for the conspiracy is itself evidence — it shows how good the cover-up is. A lack of bodies proves the deaths were faked. A lack of documents proves the documents were destroyed. A scientific consensus proves that scientists are all in on it. This inversion of normal epistemological standards makes conspiracy theories maximally resistant to empirical challenge.
The official explanation is definitionally suspect. Whatever governments, scientists, media outlets, or other institutional authorities say about an event is, by default, suspected of being part of the conspiracy. This means that the standard mechanisms of epistemic trust — appeal to expert authority, institutional verification, peer review — are neutralized. The conspiracy theorist must generate their own epistemological standards, which often privilege "doing your own research" (typically: finding online sources that confirm the theory) over credentialed expertise.
Callout Box: The "Paranoid Style" in American Politics
Historian Richard Hofstadter introduced the concept of the "paranoid style" in American politics in his 1964 Harper's essay of the same name. Hofstadter was not using "paranoid" as a clinical diagnosis but as a rhetorical and cognitive style characterized by the sense that "America has been largely taken away from them and their kind, though they are determined to try to repossess it and to prevent the final destructive act of subversion." The paranoid style, Hofstadter argued, involves "the qualities of heated exaggeration, suspiciousness, and conspiratorial fantasy." While Hofstadter situated the paranoid style primarily on the political right, subsequent scholars have found it across the political spectrum, and the concept has been criticized for dismissing legitimate grievances as pathology. The most nuanced reading of Hofstadter distinguishes between the style — which is real and consequential — and any necessary connection to specific political positions.
Section 13.6: Digital Amplification
The Algorithmic Ecosystem
Conspiracy theories have always existed. What has changed in the past two decades is the infrastructure for their dissemination. Digital platforms have transformed conspiracy theories from marginal beliefs circulating in specialist subcultures into mainstream phenomena capable of influencing elections, public health decisions, and political violence.
The mechanisms of digital amplification are several. Recommendation algorithms on platforms like YouTube are optimized for engagement — for keeping users on the platform as long as possible. Research by Guillame Chaslot, a former YouTube engineer, and others has documented what has come to be called the "rabbit hole" effect: algorithms consistently recommend increasingly extreme content because extreme content generates stronger emotional responses and thus higher engagement. A user who watches a moderate skeptical video about vaccines may be successively recommended more extreme anti-vaccination content, then adjacent health conspiracy content, then political conspiracy content.
YouTube's algorithm, specifically, has been documented in multiple studies as a powerful driver of conspiracy radicalization. Ribeiro et al. (2020) found that a significant fraction of users who engaged with far-right conspiracy content on YouTube had started in mainstream political content and been progressively recommended toward the fringe. The pathway was not driven by active search; it was driven by passive recommendation.
Discord, Telegram, and the Migration to Encrypted Platforms
When mainstream platforms began moderating conspiracy content following the 2016 election and accelerating after the Capitol insurrection of January 2021, conspiracy communities migrated to platforms with weaker moderation policies. Discord servers provided community infrastructure for conspiracy groups; Telegram channels offered broadcast capabilities to large audiences with minimal moderation; Gab and Parler provided social network functionality for communities deplatformed from Twitter and Facebook.
This migration dynamic has important implications for understanding how deplatforming affects conspiracy spread. Research findings are mixed. Some studies find that deplatforming reduces the reach of conspiracy content by removing it from the largest, most heavily trafficked platforms where it might recruit new adherents. Other studies find that deplatforming intensifies radicalization within communities by removing moderating voices and pushing members into more extreme, less diverse information environments.
The migration to encrypted platforms also poses significant challenges for researchers and journalists attempting to track conspiracy spread. Telegram channels, in particular, are difficult to monitor at scale because they operate largely outside the public social graph.
The Role of Social Networks
Beyond algorithmic recommendation, social networks amplify conspiracy theories through trust-based transmission. Research on information diffusion consistently finds that people are more likely to believe and share information that comes from trusted sources — friends, family members, community leaders. Conspiracy theories spread most effectively not through mass broadcast but through personal network transmission.
Kate Starbird's research at the University of Washington has mapped the network structure of alternative narrative communities and found them to be dense, highly connected, and resistant to information from outside the community. Within these networks, conspiracy theories circulate with high velocity because trust is high and countersignaling from non-community members is absent.

The Radicalization Pathway
Believing a conspiracy theory is not, in itself, a precursor to violence. Most people who believe in various conspiracy theories — about extraterrestrials, about corporate misconduct, about political manipulation — do not commit violent acts. The pathway from conspiracy belief to radicalized violence involves additional steps.
Research on radicalization, including work by scholars at the Global Network on Extremism and Technology (GNET) and by psychologist John Horgan, identifies several conditions that facilitate the transition from belief to action:
Moral licensing: Conspiracy theories that cast out-groups as evil rather than merely mistaken provide moral justification for extreme action.
