Case Study 1 — The Ortiz-Lindqvist Shop: Where the Sugar Actually Is
The Ortiz-Lindqvist household is an illustrative composite carried through this book — Marisol Ortiz, Erik Lindqvist, and their children Alma (7, peanut allergy) and Nico (13). The budget, the products and the label mechanics are typical; the family is constructed.
Setup
$180 a week. Four people. One of them allergic to peanuts, one of them thirteen.
Marisol came in with a question that was not the one she asked. What she asked was: "How much sugar should we be having?" What she meant was: "Are we doing this badly, and can we afford to fix it?"
So we did the shop instead of the arithmetic.
She photographed every label from one week's groceries. Forty-one packaged items. Here's what came back.
The audit
Added sugar, per typical serving as the family actually eats it:

Household total, roughly: 340 g/week of added sugar from packaged items alone, before anything anyone baked, bought at school, or ate at a friend's house.
Two things fell out immediately.
1. ⚠️ Nico alone accounted for about 40% of it. Five sports drinks and four chocolate milks — 193 g a week between them, all liquid, none of it registering as food to a thirteen-year-old who plays football twice a week and is, by his own account, "not really eating sweets."
2. The savoury and invisible items — sauce, bread, beans, marinade, ketchup — came to about 60 g a week. ⚠️ Not the biggest number, and the one nobody in the house knew existed.
The smoothie
Marisol's line was the most interesting thing in the audit, and she said it without a trace of irony:
"But the smoothie's fine — it says no added sugar."
It does say that, and it's true. The bottle contains no added sugar. It also contains roughly 26 g of total sugar from fruit purée and juice concentrate, in about 300 mL, drunk in two minutes.
⚠️ Section 18.3, in a bottle. The molecules are identical to the ones in fruit. What's been removed is the fiber, the structure, the water volume and the chewing — which is to say, everything that made the fruit different from a soda.
"No added sugar" is a true statement about the manufacturing process and it is doing misleading work about the physiology.
💡 Aha moment. ⚠️ The label is not lying. It is answering a question about provenance when the buyer is asking a question about dose rate.
This is the single most common way food labelling misleads without any false statement in it, and once you see it you'll see it everywhere: "no added sugar" (provenance), "made with real fruit" (provenance), "natural" (provenance), "plant-based" (provenance), "no artificial colours" (provenance).
None of these is a claim about quantity, and quantity is the only thing §18.5 says matters.
Nico
The hard part of the case, and the reason it's a case study rather than a shopping list.
Nico is 13, plays football twice a week, is growing, and is not overweight. The sports drinks were bought deliberately, by parents who thought they were doing the right thing — an athlete, sweating, needs replacement.
Chapter 15 already answered this. Electrolyte replacement is 🟢 supported beyond roughly 60–90 minutes of hard exercise, or in heat, or for heavy salty sweaters. ⚠️ Nico plays for 50 minutes, twice a week, in a temperate climate. The other three drinks are at home, on the sofa.
And here is the part that makes it awkward: there is nothing wrong with Nico. No symptom, no lab value, no weight problem. The case for changing anything is entirely prospective — a habit forming at 13 that will be a default at 30.
Which raises Chapter 17 §17.13's question in an unusual form: does this matter enough to intervene, when the person is a teenager and the intervention will be experienced as a rule?
What we actually did:
⚠️ Nothing was banned. Marisol's instinct was to remove the drinks; that's the intervention most likely to make them desirable and to make her the person who took them away.
Instead: the two match-day drinks stayed — deliberately, as the Project Checkpoint's Row 3, and because there's an argument for them even if it's a weak one. The three sofa drinks were not restocked, and a large water bottle with his name on it appeared in the fridge.
Nico noticed on day four and said "we don't have any of the blue ones." He was told they were expensive, which was true, and that they were for football, which he accepted. He has not raised it since.
⚠️ The chocolate milk was left entirely alone, and I want to be explicit about why: he's 13, growing, gets a meaningful share of his calcium and protein from it, and there is a version of this conversation that ends in a 13-year-old with a complicated relationship with food (Chapter 34). That risk is larger than 88 g of sugar a week.

Fifty-four percent of the added sugar removed, and the weekly shop got cheaper by nine dollars.
⚠️ Only one line cost more, and it was the yoghurt — where plain yoghurt plus frozen berries beats flavoured yoghurt on sugar, fiber and food quality, and loses on price by $1.20.
What we didn't do
This list matters as much as the other one.
No ban on anything. No mention of sugar to Alma, who is 7 and whose relationship with food already has one restriction in it that she didn't choose (Chapter 28). ⚠️ Adding a second category of forbidden food to a child who already has a life-threatening allergy is a bad trade, and Chapter 34 explains why in detail.
No calorie counting. No "sugar-free" replacements — the household bought no diet drinks, no sweetener, no sugar-free snacks. §18.13's comparator logic says a diet soda beats a regular soda; it says nothing in favour of buying one where water was already the default.
And no argument about fructose. ⚠️ Nothing in this case study depended on which side of §18.15's table is right. Both would have recommended the same six changes.
Analysis
1. Liquid was 57% of the household total — sports drinks, chocolate milk, smoothie, sweetened coffee. §18.7. And it was the part nobody experienced as eating.
2. The invisible savoury sources were real but not the biggest lever. ⚠️ Worth knowing about, worth fixing because it's cheap and easy, and not worth leading with. Sauce and marinade were 36 g of 340.
3. "No added sugar" was the most misleading true statement in the shop. ⚠️ Provenance claims answering dose questions — and there were five other provenance claims on the same forty-one items.
4. The deliberate keeps were load-bearing. Chocolate milk, match-day drinks, granola bars, fruit snacks. A 54% reduction that survives is worth more than an 85% reduction that lasts eleven days (Chapter 10 §10.4).
5. ⚠️ The intervention with a 13-year-old was designed around not being a rule. Restocking decisions, not prohibitions. Nobody said the word sugar to him.
**6. And it saved $470 a year**, which for a household on $180 a week is the part that made it sustainable — and which is the fifth chapter running where the evidenced change was the cheaper one.

Your Turn
Do the Ortiz-Lindqvist shop on your own kitchen. Photograph the labels — you'll read them differently from a photograph than from a shelf.
Step 1 — Sort every item into three columns:

Step 2 — Add up each column. ⚠️ Most people's third column is bigger than they expect and smaller than the first. Both halves of that sentence are the finding.
Step 3 — Separate liquid from solid, ignoring the columns. What percentage of your added sugar arrives as a drink? §18.7 says this is the number that matters most.
Step 4 — Find every provenance claim on your packaging. No added sugar · made with real fruit · natural · no artificial anything · plant-based · wholegrain. For each, ask: ⚠️ is this telling me about where it came from, or about how much is in it?
Step 5 — Pick your six changes, and make one of them a deliberate keep.
And a costing column, because it changes what you'll actually do:

⚠️ If your total cost column is positive, check it. In this chapter, in Chapter 11, Chapter 15 and Chapter 16, the evidenced change was the cheaper one every time. If yours isn't, you may have bought a solution rather than removed a source.
