Case Study 1 โ€” The Green Folder, Reopened: Theo's Four Headline Pairs, Answered

โš ๏ธ Theo Vasquez is the book's anchor case and an illustrative composite, established in Chapter 1. โš ๏ธ His green folder โ€” four pairs of contradictory headlines collected over eighteen months โ€” was the question this book was written to answer. This case study answers it, pair by pair.**

Setup

โš ๏ธ The folder has been in a drawer since Chapter 1. He brought it back.

โš ๏ธ His instruction to himself, written on the inside cover in Chapter 1 and unchanged:

"Work out which one is right."

โš ๏ธ That instruction turns out to be wrong, and how it is wrong is this case study's finding.

โš ๏ธ In three of the four pairs, neither headline was right. โš ๏ธ In the fourth, both were, about different questions.

Pair 1 โ€” "Eggs cause heart disease" / "Eggs are a superfood"

โš ๏ธ Six questions from ยง37.7, applied.

โš ๏ธ Question โš ๏ธ Headline A โš ๏ธ Headline B
What rung? โš ๏ธ Observational cohort โš ๏ธ Nutrient content, no outcome at all
Comparison? โš ๏ธ Against people eating fewer eggs, who differ in many ways โš ๏ธ Against nothing
Denominator? โš ๏ธ Per egg per day, which is not how anyone eats โš ๏ธ Per 100 g, a portion nobody consumes
โš ๏ธ Replaces or adjusts? โš ๏ธ Replaces โš ๏ธ Replaces
Anything sold? โš ๏ธ A book โš ๏ธ An industry body
โš ๏ธ What would disprove it? โš ๏ธ Nothing offered โš ๏ธ Nothing offered

โš ๏ธ The answer, from Chapter 9: โš ๏ธ dietary cholesterol has a much smaller effect on blood cholesterol than was assumed for decades, the guidance limiting it was withdrawn, and eggs are an inexpensive complete-protein food with a modest and unremarkable place in a diet.

โš ๏ธ Neither headline. And Theo's own reaction:

"So the actual answer is 'eggs are fine and not important.' That's not a headline."

โš ๏ธ No. ยง37.6 explains why it never was one โ€” and this is the pattern for three of his four pairs.

Pair 2 โ€” "Red meat causes cancer" / "Red meat is essential nutrition"

โš ๏ธ The most instructive of the four, because here the honest answer is a distinction rather than a verdict.

โš ๏ธ What the evidence supports โš ๏ธ Chapter
โš ๏ธ PROCESSED meat and colorectal cancer: a consistent association, classified as carcinogenic, with a real but modest absolute risk increase Ch 9, Ch 26
โš ๏ธ UNPROCESSED red meat: a weaker and less consistent association Ch 9
โš ๏ธ Red meat as a source of iron, B12, zinc and complete protein: genuine, and it matters for some populations more than others Ch 8, Ch 14, Ch 25
โš ๏ธ The absolute risk framing that both headlines omitted Ch 9's comparative-risk section

โš ๏ธ Where each headline went wrong:

โš ๏ธ Headline A conflated processed and unprocessed, and reported a relative risk without a baseline โ€” โš ๏ธ which is the single most common misreporting in nutrition and Chapter 2 ยง2.6's subject.

โš ๏ธ Headline B was true of nutrients and false as a dietary conclusion. โš ๏ธ "Contains essential nutrients" does not establish "you need this food," because Chapter 8 and Chapter 14 list the alternatives.

โš ๏ธ The honest answer: eat less processed meat specifically, unprocessed red meat is a moderate question, and neither headline was about the same thing.

Pair 3 โ€” "Fat makes you fat" / "Carbs make you fat"

โš ๏ธ The pair Theo found most frustrating, because both had books behind them.

โš ๏ธ The answer, from Chapters 4, 7, 9 and 10:

โš ๏ธ Neither macronutrient has been shown to have the special causal role each camp claims. โš ๏ธ Controlled feeding trials at matched energy find small differences. Free-living trials find that adherence dominates (Ch 10's threshold).

โš ๏ธ And the thing both books were partly right about, which is why both had converts:

โš ๏ธ Each diet works for the people who can stick to it, and each removes a large category of energy-dense food that its adherents had been eating a lot of.

๐Ÿ’ก Aha moment. โš ๏ธ "They're both descriptions of what happens when you stop eating one of the two things most of your junk food was made of."

โš ๏ธ That is close to right, and it is Chapter 10's finding in the reader's own words.

โš ๏ธ ยง37.2b's row for this pair: โš ๏ธ the insulin-carbohydrate mechanism LARGELY FAILED; the practical advice about refined carbohydrate was ABSORBED.

โš ๏ธ And DIETFITS (Ch 35 ยง35.3b) is the direct test: a year, both arms eating well, no meaningful average difference, enormous individual variation, and no predictor of who did better on which.

Pair 4 โ€” "Coffee is good for you" / "Coffee is bad for you"

โš ๏ธ The pair where both headlines were defensible, and the reason is worth understanding.

โš ๏ธ The claim โš ๏ธ Verdict
โš ๏ธ Moderate coffee is associated with lower all-cause mortality in cohorts โš ๏ธ ๐ŸŸข, and heavily confounded โ€” Ch 2's healthy-user structure applies
โš ๏ธ Caffeine is an effective ergogenic aid โš ๏ธ ๐ŸŸข (Ch 16, Ch 23)
โš ๏ธ Caffeine late in the day impairs sleep, in a dose- and person-dependent way โš ๏ธ โœ… (Ch 33 ยง33.11)
โš ๏ธ Coffee causes disease at moderate intakes โš ๏ธ ๐ŸŸ
โš ๏ธ Coffee is a health intervention worth adopting โš ๏ธ ๐ŸŸก โ€” the association is not strong enough to act on

โš ๏ธ Both headlines cited real findings about different outcomes, and neither said which outcome.

โš ๏ธ The honest answer: coffee is fine, it is not a health intervention, the timing matters more than the amount for most people, and it is a preference.

โš ๏ธ Which is ยง37.4's optional list, and is the least exciting possible resolution.

โš ๏ธ What the folder actually contained

โš ๏ธ Sorted after the fact, and this is the case study's real finding.

โš ๏ธ Of the eight headlines
โš ๏ธ Reported a real finding accurately โš ๏ธ One (the processed meat one, partially)
โš ๏ธ Reported a real finding without its comparison, baseline or denominator โš ๏ธ Four
โš ๏ธ Reported nutrient content as if it were an outcome Two
โš ๏ธ Reported a mechanism as if it were a result One
โš ๏ธ Was outright fabricated โš ๏ธ NONE

โš ๏ธ Chapter 17's finding, in one man's drawer: the problem is over-extension, not fabrication.

โš ๏ธ And the structural observation:

โš ๏ธ Every one of the eight was exciting. โš ๏ธ Not one of them said "this adjusts the existing advice slightly," and ยง37.2b says that is what the true ones almost always do.

โš ๏ธ Theo's instruction to himself โ€” work out which one is right โ€” assumed the answer was in the folder. โš ๏ธ It was not in the folder. The folder was the phenomenon.

โš ๏ธ What he does now

โš ๏ธ He kept the folder and changed what goes in it.

โš ๏ธ He no longer collects contradictory headlines. โš ๏ธ He runs ยง37.7's six questions on anything that makes him want to change what he eats, and he writes down which question it failed on.

โš ๏ธ Eighteen months of the new folder:

โš ๏ธ Claims logged โš ๏ธ 31
โš ๏ธ Failed at question 1 (rung) โš ๏ธ 14
โš ๏ธ Failed at question 2 (comparison) โš ๏ธ 6
โš ๏ธ Failed at question 3 (denominator) 3
โš ๏ธ Failed at question 4 (replaces rather than adjusts) โš ๏ธ 5
โš ๏ธ Failed at question 5 or 6 2
โš ๏ธ Survived all six and adjusted what he does โš ๏ธ 1

โš ๏ธ The one that survived: increasing pulses, on the fibre evidence (Ch 11). โš ๏ธ Which was already in the boring answer, and which he had read in Chapter 11 and not acted on until a claim survived his own filter.

โš ๏ธ That is the chapter's argument as a personal outcome. โš ๏ธ Thirty of thirty-one exciting claims failed a ten-minute check, and the one that survived was something the boring answer already contained.

Analysis

1. โš ๏ธ In three of four pairs, NEITHER headline was right. โš ๏ธ The instruction "work out which one is right" presupposed a false structure.

2. โš ๏ธ None of the eight was fabricated. โš ๏ธ Four omitted a comparison or baseline, two reported nutrient content as an outcome, one reported a mechanism as a result (Ch 17's finding, replicated in a drawer).

3. โš ๏ธ Pair 2 needed a DISTINCTION, not a verdict โ€” processed versus unprocessed โ€” and both headlines had collapsed it.

4. โš ๏ธ Pair 3's two camps were each describing the removal of a category of energy-dense food, โš ๏ธ and DIETFITS tested the mechanism directly and refuted it (Ch 35 ยง35.3b).

5. โš ๏ธ Pair 4's headlines were both defensible about different outcomes and neither named its outcome โ€” โš ๏ธ which is ยง37.7's question 3 in a different form.

6. โš ๏ธ All eight were exciting, and none said "this adjusts the existing advice slightly." โš ๏ธ ยง37.2b says the true ones almost always do.

7. โš ๏ธ Thirty-one claims over eighteen months; one survived; it was already in the boring answer.

8. And he acted on Chapter 11's advice only after it survived his own filter. โš ๏ธ Which suggests what the book was actually for, and Chapter 38 has to make that argument explicitly.

Discussion Questions

  1. โš ๏ธ "Work out which one is right" presupposed a false structure. What is the better question? โš ๏ธ Write the instruction Theo should have put on the folder in Chapter 1.

  2. โš ๏ธ None of the eight headlines was fabricated. โš ๏ธ Does that make the situation better or worse? Which is easier to defend against?

  3. โš ๏ธ Pair 2 required a distinction rather than a verdict. โš ๏ธ Find two other places in this book where the honest answer was a distinction. (Chapters 18 and 22.)

  4. โš ๏ธ Pair 3's camps both had converts and both were partly right. โš ๏ธ Is "adherence explains it" a satisfying answer? Why is it so rarely the headline?

  5. โš ๏ธ Thirty of thirty-one claims failed a ten-minute check. โš ๏ธ Is that ratio plausible, or is Theo now over-filtering? How would he tell?

  6. โš ๏ธ He acted on Chapter 11 only after it survived his own filter. โš ๏ธ What does that say about how advice becomes behaviour? (Ch 33, and Ch 35's Theo said something similar about conviction.)

Your Turn

Step 1 โ€” โš ๏ธ Find your own folder.

โš ๏ธ Two nutrition claims you have encountered that contradict each other.

A: _ B: _

Step 2 โ€” โš ๏ธ Run ยง37.7's six questions on both.

โš ๏ธ Question โš ๏ธ A โš ๏ธ B
Rung?
Comparison โ€” against what?
Denominator?
โš ๏ธ Replaces or adjusts?
Anything sold? Does it generate a rule?
โš ๏ธ What would disprove it?

Step 3 โ€” โš ๏ธ Ask whether the question was even the right one.

โš ๏ธ Are A and B about the same outcome, the same population, and the same dose?

โš ๏ธ In three of Theo's four pairs they were not.

Step 4 โ€” โš ๏ธ Classify, don't adjudicate.

โš ๏ธ Accurate ยท missing comparison or baseline ยท nutrient content as outcome ยท mechanism as result ยท fabricated.

โš ๏ธ Count how many land in the last category. For Theo it was zero.

Step 5 โ€” โš ๏ธ Start the new folder.

โš ๏ธ For the next six months, log anything that makes you want to change what you eat, and write down which of the six questions it failed on.

โš ๏ธ Expect most to fail at question 1.

Step 6 โ€” โš ๏ธ And notice what survives.

โš ๏ธ If something survives all six, check it against ยง37.3's plate before doing anything.

โš ๏ธ Theo's survivor was already in it, and the useful realization was not the claim โ€” it was that he had read it, agreed with it, and not acted on it for eighteen months.
