Case Study 1 โ€” Theo, Three Years Later: What Actually Changed

โš ๏ธ Theo Vasquez has been this book's anchor case since Chapter 1 and is an illustrative composite. โš ๏ธ His canon numbers were established in Chapter 4: RMR 1,908, TDEE 2,670, intake 3,160 kcal, fibre 14 g, added sugar 78 g, sodium 4,100 mg, A1c 5.9%. โš ๏ธ This is the last time he appears.**

The arc, before this

โš ๏ธ Ch 1 โš ๏ธ The green folder. Four contradictory headline pairs. A Belief Inventory, sealed
โš ๏ธ Ch 4 โš ๏ธ The numbers. The first time anyone had measured rather than guessed
โš ๏ธ Ch 17 โš ๏ธ The Inventory unsealed โ€” ten beliefs, eight wrong, and all four he held at confidence 5
โš ๏ธ Ch 33 โš ๏ธ The 21:40 problem diagnosed โ€” eleven years, and the cue was the end of the washing up
โš ๏ธ Ch 35 โš ๏ธ Fourteen days with a glucose monitor, and taking it off on day fourteen as planned
โš ๏ธ Ch 37 โš ๏ธ The folder answered. The new folder: 31 claims, 30 failed, one survivor

โš ๏ธ Three years from Chapter 1. This is what the file looks like now.

โš ๏ธ The numbers

โš ๏ธ Chapter 4 โš ๏ธ Now
Fibre โš ๏ธ 14 g โš ๏ธ ~31 g
Added sugar โš ๏ธ 78 g โš ๏ธ ~30 g
Sodium โš ๏ธ 4,100 mg โš ๏ธ ~2,900 mg
โš ๏ธ Alcohol โš ๏ธ 6% of intake โš ๏ธ ~2%
A1c โš ๏ธ 5.9% โš ๏ธ 5.5%
โš ๏ธ Supplements โš ๏ธ 4 โš ๏ธ 0
โš ๏ธ Money on nutrition products โš ๏ธ ~$700/yr** | โš ๏ธ **~$40, once, and he regretted it

โš ๏ธ And the number he cares about, which is not on that list:

"I don't think about food that much any more. That's the actual difference."

โš ๏ธ What produced the changes, honestly attributed

โš ๏ธ This is the part worth doing carefully, because a case study that credits the book for everything would be exactly what ยง38.2 warned about.

โš ๏ธ The change โš ๏ธ What actually caused it
โš ๏ธ Fibre 14 โ†’ 31 g โš ๏ธ Chapter 11, eighteen months later, after it survived his own filter (Ch 37 CS1)
โš ๏ธ Added sugar down โš ๏ธ Mostly one decision: he stopped buying soft drinks (Ch 18)
โš ๏ธ Sodium down โš ๏ธ A side effect of cooking more, not a target (Ch 15, Ch 36)
โš ๏ธ Alcohol down โš ๏ธ Ch 12 gave him the number, and a change in his social life did the rest
โš ๏ธ The 21:40 pattern โš ๏ธ Ch 33's cue diagnosis โ€” and the tea, which is not a nutritional intervention
โš ๏ธ A1c โš ๏ธ Probably fibre, weight, alcohol and walking together. He cannot separate them and neither can I
โš ๏ธ Four supplements to zero โš ๏ธ Chapter 16, and it was the fastest change he made
โš ๏ธ Not buying things โš ๏ธ Chapter 35, and thirty declined claims (Ch 37 CS1)

โš ๏ธ Two attributions that are NOT to this book:

โš ๏ธ The walking came from a dog. โš ๏ธ The eating-with-people came from a flatmate who cooks on Wednesdays.

โš ๏ธ Both are in Chapter 37's one-page answer and neither was adopted because it was in Chapter 37's one-page answer.

โš ๏ธ Which is a fair description of how most dietary change actually happens, and it is worth a book about nutrition admitting it.

โš ๏ธ What did NOT change

โš ๏ธ Because three years of improvement listed without the failures is advertising.

โš ๏ธ He still does not eat oily fish twice a week. โš ๏ธ He has read Chapter 19, agrees with it, buys tinned sardines occasionally, and mostly does not. โš ๏ธ No mechanism, no obstacle, no explanation. He just doesn't.

โš ๏ธ He still eats badly for about a week when work is bad. โš ๏ธ The bad-night meal absorbs some of it. The rest is not absorbed.

โš ๏ธ His vegetable intake is adequate rather than good, โš ๏ธ and he knows it, and has known it for three years.

โš ๏ธ And he has never written the two-page framework in ยง38.6. โš ๏ธ He was asked to. He agreed it was a good idea. He has not done it.

โš ๏ธ That last one should be read as data about this chapter rather than about him.

โš ๏ธ The Belief Inventory, third pass

โš ๏ธ Sealed in Chapter 1. Unsealed in Chapter 17, where eight of ten were wrong and all four confidence-5 beliefs were among them.

โš ๏ธ He redid it. Ten new beliefs, rated for confidence, then checked against the book.

โš ๏ธ Ch 1 โš ๏ธ Now
โš ๏ธ Beliefs rated confidence 5 โš ๏ธ 4 โš ๏ธ 1
โš ๏ธ Of those, wrong โš ๏ธ 4 of 4 โš ๏ธ 0 of 1
โš ๏ธ Beliefs rated 3 (unsure) โš ๏ธ 1 โš ๏ธ 6
โš ๏ธ Total wrong โš ๏ธ 8 of 10 โš ๏ธ 2 of 10

โš ๏ธ The important row is the third one, not the last one.

โš ๏ธ He did not mainly become more right. โš ๏ธ He became much less certain โ€” from one "unsure" to six.

๐Ÿ’ก Aha moment. โš ๏ธ "I used to have opinions about food. Now I mostly have questions and about four things I'd argue for."

โš ๏ธ That is calibration, and it is ยง38.3's second capability.

โš ๏ธ The one belief he still holds at confidence 5: โš ๏ธ "Most of what gets sold to me about food is nonsense."

โš ๏ธ Which is, per ยง38.5, supported by roughly 133 โŒ and ๐ŸŸ  verdicts โ€” and is also the belief most likely to make him dismiss something true. โš ๏ธ He knows that, which is the useful part.

โš ๏ธ The folder

โš ๏ธ It is still in the drawer. He has not thrown it away.

โš ๏ธ The note on the inside cover has been crossed out and rewritten:

โš ๏ธ ~~Work out which one is right.~~

โš ๏ธ Work out what kind of claim it is.

โš ๏ธ His account of the difference:

"The first one assumes there's an answer in the folder. There isn't. The folder is just what the internet does. The second one is something I can actually do, in about ten minutes, on anything."

โš ๏ธ That is ยง38.2's argument, arrived at independently by the reader it was written about, and it is the closest thing to evidence this chapter has.

Analysis

1. โš ๏ธ The measurable changes are real and modest โ€” โš ๏ธ fibre, sugar, alcohol, sodium, A1c, four supplements to zero โ€” and accumulated over three years rather than in a transformation.

2. โš ๏ธ The largest single financial effect was the things he stopped buying (ยง38.5, ยง38.10).

3. โš ๏ธ Two of the changes in Chapter 37's one-page answer came from a dog and a flatmate, โš ๏ธ not from the book โ€” which is how most dietary change happens and is worth admitting.

4. โš ๏ธ The fibre change took eighteen months and required the claim to survive his own filter, โš ๏ธ not to be told to him again (Ch 37 CS1).

5. โš ๏ธ He has not written the framework, and the framework is this chapter's central instruction. โš ๏ธ That is a finding about ยง38.6, not about him.

6. โš ๏ธ The Belief Inventory shows CALIBRATION rather than correction โ€” โš ๏ธ from one "unsure" to six. โš ๏ธ He became less certain far more than he became more right.

7. โš ๏ธ His one remaining confidence-5 belief is the one most likely to make him wrong, โš ๏ธ and he can say so.

8. And "I don't think about food that much any more" is the outcome he names first. โš ๏ธ It is not a nutritional endpoint, it is not measurable, and it is probably the honest deliverable of thirty-eight chapters.

Discussion Questions

  1. โš ๏ธ The fibre change took eighteen months and required his own filter rather than another telling. โš ๏ธ What does that imply about how nutrition education should be designed?

  2. โš ๏ธ A dog and a flatmate produced two of the changes. โš ๏ธ How much of what looks like dietary knowledge working is actually circumstance? How would you tell?

  3. โš ๏ธ He never wrote the framework. โš ๏ธ Is ยง38.6 therefore bad advice, or is it advice for a different reader? Which reader?

  4. โš ๏ธ He became less certain far more than he became more right. โš ๏ธ Is that a good outcome for a textbook? Argue both sides.

  5. โš ๏ธ "Most of what gets sold to me about food is nonsense" is both well supported and the belief most likely to make him dismiss something true. โš ๏ธ How do you hold a justified generalization without it becoming a wall? (ยง38.7's last caution.)

  6. โš ๏ธ "I don't think about food that much any more." โš ๏ธ Should that be the goal of a nutrition book? What would be lost if it were?

Your Turn

Step 1 โ€” โš ๏ธ Write your own inventory. Ten beliefs about food. Rate each 1โ€“5.

โš ๏ธ Do it before you check anything. The rating is the exercise.

Step 2 โ€” โš ๏ธ Check them against this book.

โš ๏ธ Belief โš ๏ธ My confidence โš ๏ธ Verdict โš ๏ธ Gap

Step 3 โ€” โš ๏ธ Count the RIGHT thing.

โš ๏ธ Not how many you got right. โš ๏ธ How many you rated 5 and got wrong, and how many you rated 3.

โš ๏ธ Theo went from one "unsure" to six. That is the movement that matters.

Step 4 โ€” โš ๏ธ Find your confidence-5 belief that is most likely to make you wrong.

โš ๏ธ Everyone has one. It is usually the one that feels like sophistication rather than belief.

Step 5 โ€” โš ๏ธ Rewrite your own note.

โš ๏ธ What instruction were you carrying when you started this book? _ โš ๏ธ What would you write now? _

Step 6 โ€” โš ๏ธ And attribute honestly.

โš ๏ธ Of anything you have changed while reading this: how much was the book, and how much was a dog, a flatmate, a new job or a change you were going to make anyway?

โš ๏ธ The honest answer is usually humbling, and it is the same answer ยง38.2 gave about the book's own effect.
