So is sugar bad? is not a question. Neither is is fat bad?, are carbs bad?, is coffee good? They are category errors, which is why decades of evidence haven't settled them.

The Chemistry, briefly

Sucrose = glucose + fructose, bonded, cleaved in the small intestine within minutes. HFCS-55 = ~55% fructose / ~42% glucose, free. โš ๏ธ By the liver, near-indistinguishable. Agave = โš ๏ธ often 70โ€“90% fructose โ€” usually the highest-fructose product in the shop.

Two honest qualifications on DNL: it's a modest contributor to whole-body fat balance at ordinary intakes (most body fat was eaten as fat โ€” Ch 6 ยง6.7); and โš ๏ธ most high-dose fructose studies are also overfeeding studies, so they can't separate Claim 2 from Claim 3. At matched calories the differences shrink substantially โ€” whether to zero is exactly what's unresolved.

People do not compensate for liquid calories. Eat 200 kcal solid โ†’ you eat somewhat less later. Drink 200 kcal โ†’ compensation is close to zero.

Why: faster gastric emptying ยท less distension ยท weaker satiety hormone response (Ch 3 ยง3.6) ยท no chewing ยท consumed alongside other activity rather than as an eating event.

โš ๏ธ Theo drank two sodas a day and did not experience himself as having eaten anything. Nobody fails to notice a doughnut.

ยง18.6 โ€” Where the alcohol analogy holds and breaks

Holds: hepatic metabolism ยท steatosis at dose ยท liquid, poorly compensated, heavily marketed ยท NAFLD is a real and growing problem.

Breaks: โš ๏ธ ethanol produces acetaldehyde, a direct cellular toxin โ€” fructose has no counterpart ยท the intestine buffers fructose and does not buffer ethanol, so the low-dose curves differ in shape ยท ethanol is psychoactive and causally implicated in injury and death ยท โš ๏ธ Chapter 12 had ALDH2 as a causal instrument; sugar has no equivalent.

Rejecting the analogy is not the same as rejecting the concern.

Documented (Kearns, Glantz & Schmidt, JAMA Internal Medicine, 2016, from recovered correspondence): the Sugar Research Foundation funded a 1967 NEJM review that downplayed sugar and emphasized fat, with undisclosed funding and sponsor input. โœ… Real.

Over-extended: that it single-handedly caused the low-fat era. โš ๏ธ Four qualifications: non-disclosure was normal in 1967 ยท Keys's work was independent ยท Yudkin's suppression story is embellished ยท the low-fat era had many parents (Ch 9 ยง9.7).

โš ๏ธ This looks exactly like the suppressed-truth shape Chapter 17 taught you to dismiss โ€” and it's true.

The test: a suppressed-truth claim is unfalsifiable and cites nothing. A documented episode names the year, the journal, the authors, the sponsor, and the archive. Carry this into Ch 19 and Ch 20.


ยง18.13 โ€” Sweeteners, and a structure worth learning

โš ๏ธ 2023: IARC classified aspartame Group 2B ("possibly carcinogenic"); JECFA reaffirmed the ADI of 40 mg/kg/day. Same week, reported as contradictory. They answered different questions:

On weight: WHO 2023 issued a conditional recommendation against NSS for weight control, on low-certainty, largely observational evidence with obvious reverse causation. RCTs substituting NSS for sugar generally show modest benefit. โš ๏ธ The trials and the cohorts disagree and the WHO weighted the cohorts.

Verdict ๐ŸŸก โ€” and the comparator is the whole question: versus soda, likely an improvement. Versus water, no case.


What To Actually Do
